“Should this application be approved and it is determined that there is a payer source for my services; hospital charity will only cover the remaining balance after payment from the payer source.”

“If you have no health insurance and/or limited financial resources, you may be eligible for free or discounted services.”

“Quiero solicitar asistencia financiera.
“No, no tengo seguro médico.”
“No puedo pagar hoy.

Regional context: Upstate

Covers Greenville, Spartanburg, Anderson, Pickens, Oconee, Laurens, Cherokee, Union, Greenwood and Abbeville counties.

What stands out for patients

Prisma is the most generous in the region: free up to 250% FPL and 77% off up to 400%, for uninsured patients and insured patients in its network. But its policy excludes health care sharing ministries that reimburse the patient, and a policy that pays you could be treated the same way.

Spartanburg Regional is a public hospital (a state health district). It only publishes a two-page summary: free up to 200% FPL, with partial help above that. Its application asks whether you're a U.S. citizen.

Bon Secours St. Francis helps insured or uninsured patients: free up to 200% FPL, 81% off up to 400%, and deductibles and copays go on the sliding scale. If you may qualify for Medicaid, you must apply first.

Self Regional (Greenwood) requires legal South Carolina residency and an application for a subsidized Marketplace plan. Free up to 200% FPL, with an asset limit of $6,000 ($8,000 for families).

AnMed (Anderson and Cannon): free up to 200% FPL, 85% off up to 300% and 75% up to 400%, for South Carolina or Georgia residents only. For outpatient or emergency care under $40,000, it screens uninsured patients automatically.

Abbeville uses Self Regional's program (legal South Carolina residency, up to 200% FPL) and asks for Social Security cards for the whole household.

Shriners Children's treats children regardless of ability to pay or insurance.

How long an approval lasts

Spartanburg Regional
The published summary doesn't say how long an approval lasts.

Requirements

Who qualifies

Free up to 200% FPL (for example, under $31,300 a year for one person). Above that there may be partial help; a financial counselor works out how much.

What to bring

The signed application listing everyone in your household, with proof of all income (no income information means automatic denial); if you own a business, your personal and business tax returns (Schedule C or K); and your housing details (mortgage or rent).
Residency / immigration
Not stated; the application asks whether you're a U.S. citizen
Watch out
You must provide the information needed to apply for Medicaid or Medicare. It's a public hospital (a state health district): talk to an immigration attorney if you may apply for a green card. The full policy isn't published. The application asks for a Social Security number and whether you're a U.S. citizen. If another payer covers your care, assistance covers only what's left after it pays.

Accident and hospital indemnity policies

Applied to bill

At Spartanburg Regional, having an accident or hospital indemnity policy doesn’t by itself stop you from qualifying. Expect the hospital to apply what the policy pays to your bill first; the rest of the bill is judged by your income. If the hospital asks about other insurance, benefits or income, tell them about your policy.

Health plan: out-of-pocket assistance: Hospital decides. Spartanburg Regional’s policy doesn’t say whether its financial assistance helps with deductibles and copays, so the hospital decides.

“Should this application be approved and it is determined that there is a payer source for my services; hospital charity will only cover the remaining balance after payment from the payer source.”

“If you have no health insurance and/or limited financial resources, you may be eligible for free or discounted services.”

How to apply

When

At any point in your care. The decision comes in writing, usually within 30 days.
Pre-screening for planned care
Not stated (you must provide what's needed to apply for Medicaid or Medicare)
Say“I'd like to apply for the Spartanburg Regional Financial Assistance Program. Please give me the application.”

How to ask for financial assistance at each Upstate hospital

For every hospital

No particular words make a patient qualify. Eligibility depends on income, household size, residency and sometimes immigration status, and patients must answer truthfully. What the right words do is make sure an eligible patient asks for the correct program, at the right time, without giving up help by accident. Read this before you need care.

Ask first, pay later
“I'd like to apply for financial assistance. Please give me the application and the plain-language summary.”
If asked whether you have health insurance, and you don’t have Marketplace or employer qualifying coverage
“No, I don't have health insurance.” Why this is true: health insurance pays the hospital for your care. Accident, hospital indemnity and specified-disease policies pay you a fixed amount of cash, no matter what the bill is. Federal law (HIPAA and the ACA) treats them as “excepted benefits,” not health insurance. A discount medical program isn't insurance either.
If asked about other insurance, benefits or income
“I have a policy that pays me, not the hospital, for example if I have an accident.” This applies to any question or form about other insurance, coverage, benefits, sources of payment or income. If they ask for details, give the policy name and what it pays.
If asked to pay or take a discount
“I can't pay today. Please screen me for financial assistance before I agree to any payment or discount.”
At discharge
“Has my financial assistance screening started? What's the deadline, and where do I send my documents?”
Don't
Pay with a credit card, sign up for medical financing, or sign a payment plan before applying.
Immigration questions
Never delay emergency care. If you have a green card application ahead, an immigration attorney or accredited representative can tell you whether using a program could affect it.
